PHARMACODYNAMICS: ZIDOCINDHG is a combination of spiramycin macrolide and metronidazole antibiotic family 5-nitro-imidazol, used to treat oral infections. Spiramycin has antibacterial activity on bacteria that often cause oral diseases such as non-group D streptococci, pneumococcal bacteria, Mycoplasma, Chlamydia, Corynebacterium, Actinomyces. Antimicrobial activity of metronidazole on bacteria that often cause oral diseases such as mandatory anaerobic bacilli: Clostridium, Bacteroides fragilis, Peptostreptococcus, Peptococcus, C. perfringens, Bifidobacterium bifidum.
PHARMACOKINETICS: Spiramycin is not fully absorbed from the gastrointestinal tract. Oral absorption is about 20 - 50% of the dose. The average elimination half-life is 5 - 8 hours. Drugs mainly excreted in bile. Metronidazole is usually rapidly and completely absorbed after oral administration. The elimination half-life is about 8 hours. Metronidazole metabolized in the liver into metabolites hydroxy and acid, excreted in the urine partly as glucuronide. The two active ingredients spiramycin and metronidazole in ZIDOCINDHG penetrate well into the tissues of the oral cavity: in saliva, gums and alveolar bone.
INDICATIONS: Acute, chronic or recurrent oral infections especially tooth abscess, gingivitis, stomatitis, periodontitis, parotid gland inflammation, inflammation of the lower jaw. Prevention of postoperative dental infections.
CONTRAINDICATIONS: Hypersensitivity to spiramycin or metronidazole. Children under 6 years old.
PRECAUTIONS: For patients with liver dysfunction, because the drug can cause liver toxicity. Do not drink alcohol while taking the drug.
Blood counts should be checked during long-term treatment.
PREGNANT WOMEN AND LACTATION: Should not be used in the first trimester of pregnancy. Should stop breastfeeding while taking the drug.
DRIVING AND OPERATION OF MACHINERY: Caution when driving and operating machinery.
DRUG INTERACTIONS:
For spiramycin: Concomitant administration with oral contraceptives counteracts conception prevention.
For metronidazole: Increases the effect of anticoagulants (warfarin) and non-depolarizing muscle relaxants (vecuronium). Use with alcohol disulfiram type reaction. Increases the concentration of lithium in the serum. Phenobarbital increases metronidazole metabolism.
UNWANTED EFFECTS:
Gastrointestinal disorders: Stomach pain, nausea, vomiting, diarrhea.
Allergic reactions: Hives.
Side effects associated with metronidazole such as: metallic taste in mouth, glossitis, stomatitis, moderate leukopenia, dark urine.
Inform your doctor unwanted effects encountered when using the drug.
OVERDOSE AND TREATMENT:
Overdosage and treatment of spiramycin: No data found.
Overdose and treatment of metronidazole: Metronidazole oral as a single dose up to 15 g has been reported. Symptoms include nausea, vomiting and ataxia. Neurotoxic effects including convulsions, peripheral neuritis have been reported after 5 to 7 days at a dose of 6 - 10.4 g every 2 days. Treatment: There is no specific antidote. Symptomatic treatment.
DOSAGE AND ADMINISTRATION:
Adults: 4-6 pills / day, divided into 2-3 doses.
In severe cases, 8 tablets daily may be used.
Children from 10-15 years: 3 capsules / day.
Children from 6 - 10 years old: 2 capsules daily.
